Our COVID-19 Vaccine Clinic Hours are from 8:30am-11:00am & 1:15-4:00pm Monday – Friday.
Some require appointments.

Johnson & Johnson 1st Dose

  • No appointment necessary Monday - Friday 8:30am – 11:00am & 1:15pm – 4:00pm.
  • Must be at least 18 years of age.
  • No second dose necessary.
  • Neither 2nd doses nor boosters are yet approved for this vaccine.

 

Moderna 1st & 2nd Dose

  • Appointment required on Wednesdays only.
  • Must be at least 18 years of age.
  • 1st and 2nd doses must be booked separately.
  • 2nd dose must be scheduled no sooner than 4 weeks from your 1st dose.
  • 2nd dose must be the same brand as your previous dose.
  • 2nd dose recipients, please bring your COVID-19 Vaccine Card.

 

Moderna 3rd Dose for the moderately to severely immunocompromised

  • Must qualify under at least one of the following conditions:
    • Been receiving active cancer treatment for tumors or cancers of the blood
    • Received an organ transplant and are taking medicine to suppress the immune system
    • Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system
    • Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)
    • Advanced or untreated HIV infection
    • Active treatment with high-dose corticosteroids or other drugs that may suppress your immune response
  • Must be at least 18 years of age.
  • 3rd dose must be scheduled no sooner than 28 days from your 2nd
  • Must have previously received 2 doses of Moderna OR Pfizer
  • Please bring your COVID-19 Vaccine Card.

 

Moderna Booster Dose for the elderly and high risk individuals

  • This booster not yet approved. We will update this information as guidance changes.

 

Pfizer 1st & 2nd Dose

  • Appointment required on Mondays only.
  • Must be at least 12 years of age. Parents/Guardians must accompany minor children.
  • 1st and 2nd doses must be booked separately.
  • 2nd dose must be scheduled no sooner than 3 weeks from your 1st dose.
  • 2nd dose must be the same brand as your previous dose.
  • 2nd dose recipients, please bring your COVID-19 Vaccine Card.

 

Pfizer 3rd Dose for the moderately to severely immunocompromised

  • Must qualify under at least one of the following conditions:
    • Been receiving active cancer treatment for tumors or cancers of the blood
    • Received an organ transplant and are taking medicine to suppress the immune system
    • Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system
    • Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)

  • Must be at least 12 years of age. Proof of consent must be provided for those under 18.
  • 3rd dose must be scheduled no sooner than 28 days from your 2nd
  • Must have previously received 2 doses of Moderna OR Pfizer
  • Please bring your COVID-19 Vaccine Card.

 

Pfizer Booster Dose for the elderly and high risk individuals

  • Must qualify under at least one of the following conditions:
    • 65 years of age and older and/or residents in long-term care settings
    • 18–64 years of age with underlying medical conditions
    • 18-64 years of age who are at increased risk for COVID-19 exposure and transmission because of occupational or institutional setting (e.g. frontline medical workers, teachers, and first responders) 
  • Booster dose must be scheduled no sooner than 6 months after your 2nd dose.
  • Must have previously received the Pfizer vaccine for your 1st & 2nd dose
  • Please bring your COVID-19 Vaccine Card.

BEFORE YOU BOOK:
  • Ensure it has been at least 14 days since contact to any positive COVID-19 patients.
  • Ensure it has been at least 90 days after receiving monoclonal antibodies or convalescent plasma for treatment.
  • Bring with you to your appointment:
    • PHOTO ID
    • Insurance card(s).
    • Medicare; please bring the Red/White/Blue card. The vaccine is at no cost, our agency bills the administration fee to your insurance. If you do not have insurance the administration fee is at no charge.
    • COVID-19 Vaccination Card (if you have previously received a COVID-19 vaccination).
  • Wear a mask and a short sleeve shirt to your appointment.
  • Do not arrive more than 10 minutes before your scheduled appointment time.
  • All vaccine recipients will be required to wait 15-30 minutes after receiving their shot before leaving.
